8 RJC FH to Shri.Nikhilbhai Parmeshwarbhai  Kidi at HT-1 S/dn 24.04.12 FHO 11kv Udhyognagar feeder passing behind Kothariya colony quarter No 536, One mango tree is nearer to 11kv line, and Victim had tried to collect mangoes by climbing on mango tree, unfortunately while plucking mangoes, tree branches may be bend & came in contact with HT line conductor, due to this  victim may be came in induction zone of 11kv line and hence fatal accident occurred to Shri Nikhilbhai Parmeshwarbhai Kidi. This Human fatal accident took place on date 24/04/2012 at around 16:00 hrs. Dt:15.09.2012,Due to not attended periodical maintence i.e. Tree Cutting etc by PGVCL. 9 RJC F A to  Cow of Devuben Punjabhai Rabari at AJI-1S/dn. 16.06.12 FA There is a girder pole(LAT/DT-25/K/2) on which 2 Phase 4 Wire LT Line is existing.but on Pole (LAT/DT-25/K/3) which is nearer to that girder pole on which one street light fixture is damage (burnt) which is touched to the LT Guarding which is run through out in circuit,hence street light leakege phase flowing in to the girder pole throgh guarding,Unfortuneltly cow may came in contact with this girder pole and got electrocuted and met with the fatal accident Report yet not received NA NA Required maintence carried out by RMC

35 RJR JETPUR, CALF OF Jagmal Devanand Makrani 14.06.12 FA A calf was 4 years old which came & touch to the iron greel of gate of Sh Harshidhi factory & greel is touched to the iron angle of cement roof & whole factory roof was supported by iron angle & the wiring of the whole factory deteriorated & fans & tubes are mounted on angle which is supported roof. The calf have goat the electric leakage return current due to factory internal wiring fault. When the main switch of factory load side off then the leakage current gone out. 38 RJR Varjang Jaliya, Upleta Rural s/dn, Mohanbhai Gordhanbhai Dhami  04.07.12  FA 2 phase 3 wire LT overhead line on PSC pole of  DT no.10 (School Valu TC, 100 KVA) passing near primary school. Due to heavy rain and wind, tree branch was broken and fallen on  conductor between LT Pole location no. NIK/DT.10/C-1/009 and NIK/DT.10/C-1/010.  Due to collision of tree branch to conductor, the binding of conductor with LT Shackle insulator of LT pole location no. NIK/DT.10/C-1/010 was broken and the live conductor touched “C” clamp. The pole and surroundings land nearer to said pole is wet due to heavy rain fall. Thus the current leaked from wet pole due to rain buffalo passing nearer from the pole and came in direct contact  with pole and got an electrocuted. This is for your kind information please.

On investigating it was learnt that SMT Janakba while trying  to put wet cloths on 16 gauge M S wire fixed for drying cloths whose one end was fixed on angle on which G.I wire of S/L was tied and the other end was fixed with a nail on the wall of their house.On checking the internal wiring the phase and the neutral wire  were found wrongly connected and in all the three pin plug earth terminal live current was found On switching off the main switch of the consumer no leakage  was found. 26 SNR FH to Shri Balvantbhai Ratilalbhai at vill: Ratanpar, Ta: Wadhwan under Joravarnagar SDN, SNR-1 DN 04.07.12 FHO An accident due to leakage of current in the internal wiring NARANPARA ROAD,NR-MUNCIPALITY SCHOOL NO-1,Ratanpar had occurred on 4-7-12 SMT JANAKBA MANGALSIGH RATHODE aged 55 years got injuries due to the shock and his son RATHODE VANRAJSINGH MANGALSINGH   & Other NEIGHBOUR JOSHI BALVANTBHAI(DILIPBHAI) RATILAL trying to help his Mother also got shock & died, On visiting the site. On investigating it was learnt that SMT Janakba while trying  to put wet cloths on 16 gauge M S wire fixed for drying cloths whose one end was fixed on angle on which G.I wire of S/L was tied and the other end was fixed with a nail on the wall of their house.On checking the internal wiring the phase and the neutral wire  were found wrongly connected and in all the three pin plug earth terminal live current was found On switching off the main switch of the consumer no leakage  was found. The live phase current passed through GI of PSC pole and GI of pole is touched with streetlight angle. And with this streetlight angle Dish cable was bound with GI wire and the live phase current through this GI Dish cable is passing Through two nos PSC pole and two nos telephone iron pole. Out of this pole at PSC pole No. KCM/DT-007/C!-005 live GI of dish cable is in contact with pole GI which is grounded.
