Also conductor stringing in good condition at said location. At the time of incident victim was unloading of large size damaged glass from rickshaw near 11kv line. So during unloading this glass he might be came in induction zone. So due to this reason victim may be fall down and met with non-fatal electrical accident. Detail investigation follows.

An Electrical animal fatal accident occurred in network of LT line of Narasangpara, Rajkot of bedinaka subdivision. At that time Light Rain in surrounding area the soil got wet. At time of location, Cow was passing near the LT Pole no: 3/543:HOS/DT01/P and due to nearest pole service wire of consumer burned and phase of wire is open which is in touch with guard wire through guard angle to earth wire of LT Pole no: 3/543: HOS/DT01/P. Also pole having earth pipe but open at ground level instead ground closing. When Cow was passing near to this pole and might have touch with earth wire of LT pole and might have been got electric shock and electrocuted resulting into fatal animal accident. An Electrical animal fatal accident occurred in network of LT line of Khatkivas,Rajkot of pradhyumannagar subdivision. At that time rain in surrounding area, the soil got wet. At time of location, Calves was tie through rope with LT line girder pole MOT/DT-056/C1007 by owner. On this pole, Insulation of LT AB Cable damage which might be due to kite thread and open part of cable touch with messenger wire through on LT cross arm to girder pole MOT/DT-056/C1007.So that leakage current pass to LT Pole no: MOT/DT-056/C1007. So Calves was tied with this pole have touch with LT girder pole they got electric shock and electrocuted resulting into fatal animal accident. Detail investigation is under process.

After visiting at site it has come to notice that illegal connection of streetlight was directly connected from LT AB Cable on PSC Pole. Switch was also affixed on PSC Pole by means of 3 metres red coloured 2.5 sq mm private flexible wire. Insulation of flexible wire is found open & deteriorated. Now, open phase of this red coloured flexible wire was touching the PSC Pole fabrication due to heavy wind pressure and due to rainfall surface of PSC Pole was wet. Animals might came in contact with wet PSC Pole and due to wet surface leakage current may passed through body of animals and received Electric Shock and accident took place.

As per site visit and statement of eyewitness the victim unauthorised try to hang the halogen lamp with support 11 KV line wire. A halogen lamp was connected with two core wire. This wires hang with the 11 KV Kharachiya Hanuman JGY feeder wires for getting power supply from temple for showing magic shoe in public. When victim was pulling this wire to connect in the plug of temple at that time metal part of Halogen holder touched with live 11 KV. So the victim got electric shocked of 11 KV current. Thus fatal human accident occurred. During site visit found sparking spot on the ground near temple. The spark spot is found on halogen lamp holder and other end of wires is also burnt. 11 KV Kharachiya Hanuman JGY feeder was not tripped. Also unauthorized temple connection service wire disconnected.

This accident occurred on date 03.06.2018 but information regarding this accident is received on dated 18.06.18 vide letter No. 908 at 6:30 PM from Atkot Police Station. As per statement of eye witness this accident was occurred due to victim was doing unauthorized work on 5 KVA xmer center that time he was accidentally came in contact with live 11 KV wire and he got electrical shock. During the site visit there was burn skin found on conductor. As per 132 KV Jasdan SS record on the date 03.06.2018 in the 11 KV Pratappur JGY feeder no any trapping and sustain fault occurred. Statement of victim is pending due to he was hospitalized in Rajkot.

At that time one buffalo grazing the grass near the field of Shree Mohanbha Karshanbhai Bhut and came in contact with live LT line conductor and got electrocuted. As per eye witness this accident occurred when victim was loading cotton in the truck & standing beneath the 11 KV line, victim climb on roof of the truck cabin and he was removing plastic cover at that time he accidently came in induction zone off 11 KV Kharachiya JGY wire and may got electric shock and he fell down from roof of truck cabin. He is hospitalized at Gondal. During site visit vertical distance of the 11 KV line of Kharachiya JGY measured from the ground is 6 meter which was in proper manner, As per Mota Dadva S/s record 11 KV Kharachiya JGY feeder was not tripped at the time of accident. After receiving telephonic information from villagers of movaiya village on dated 29.06.2018 11:00 PM on the CUG mobile of SDO Paddhari. SDO Paddhari immediately sent line staff reached the site within 10 Minutes. It was found that from distribution box one line goes to circuit pole and other 6 mm2 black - cable to other pole for six nos. of consumers. The 6 mm2 cable insulation got damaged and damaged part of the cable touched with Guy-wire of circuit pole below stay insulator. The leakage current pass through the guy-wire, and the land was wet due to rainy weather, when four nos. of bullocks passes near to the guy-wire and got electric shock, due to leakage current and met with fatal accident. The line staff switch off the power supply by removing 6 mm2 cable from distribution box, as this 6 mm2 cable was feeding power to only six numbers of consumers. After rectification of damaged part of the cable, power sully to six Nos. of consumers was restored.

This accident occurred at Sonarvav agricultural area of Dhoraji in 11 KV Bhandar Ag feeder on dt. 04.07.18 under Dhoraji Rural Sdn. Primarily found that 10 KVA TC (DT115/216/013) in 11 KV Bhadar Ag feeder was erected for AG connection in name of Shree Jamanbhai Kalabhai Vaishnav connection no. 33628/01796/3. As per land owner statement accident occurred to victim, who climbed on terrace of meter room for cleaning work, 11 KV line of Bhadar Ag feeder which is passing nearby to terrace wall. One pole of 11 KV line is erected nearby meter room which is inclined towards meter room terrace due to water lodging of Doriya (Canal) nearer to live wire and came in contact with live wire of 11 KV line which is passing parallel to terrace wall and got electric shock hence non fatal electric accident occur, Further detail investigation under process. Information received telephonically on mid night of Dt: 12.07.218 from EA-Virpur to DE RSD-2, Accordingly, DE RSD-2 sent L.I. Sh.Rathva on site at 10:30 PM and DE of Gondal Rural S/Dn-2 visited site on 13.07.2018 at 08:00 AM. It is found that during heavy rain in Virpur Village, to protect himself from heavy rain, cows rushed behind the wall of house near to Pole of “Maruti Gausala”, On Said Pole, Earthing wire is covered with PVC Rigid pipe and in order, but one street light fixture was broken and that fixture came out from holding stand of street light near “Maruti Gausala”. The phase wire of street light fixture was remain connected to L.T. Line wire, but neutral wire of fixture broken from L.T. Line neutral wire and touch the G.I.Wire of earthing, The leakage current from neutral wire of fixture flows to ground through G.I. wire and as the land was wet, when four nos. of cows passes near the pole got electric shock and died at site.

Victim was trying to clear the blockage in water outlet of their terrace by 3.5 meter long iron rod. Anyhow iron rod came in contact with 11kv line of Upleta1 feeder. Accident occurred in the Ag connection of Sh Kantilal Narshibhaibhai Dhaduk, Conn.No.33823002295, 15 HP, A-1 Cat. Victim while inserting fuse grip in fuse board at that time he came in contact with upper live part of fuse lug in incoming side (service side), so victim got electric shock and died. There was no protective device provided like ELCB/RCCB, hence connection was disconnected.
Previously, in PAR submitted on 04.08.18, accident shown in private premises. But on detail investigation, it has come to known that accident occurred in network as victim came in contact with the live part of his Ag connection service incoming fuse lug.

GIDC on 18'08'2018 to DE RSD-1, Accordingly, DE RSD-1 rushed at Site, that leakage It was known currant was flowing in earthling wire of transformer neutral due to current in B phase because of faulty internal premises wiring of M/S Arrow stamping IND, consumer No 33146004739. The surrounding area near to transformer center was wet due to rain, The Buffalo was passing under the said transformer center and got electric shock and died. After switching off the MCB switch of said consumer, Leakage current stops flowing in earthling wire of said transformer center, The connection was disconnected by sub division Due to defective road side wiring of M/s Arrow stamping.

rushed at Site. It was known that some potential(Voltage) in Telephone Tubular pole due to leakage current flows in G I Wire of TV Network Cable which was tighten on the same
Pole. The Cable of TV Cable Network was coming from at LT Line Pole through G I Wire, But LT Line Pole PGVCL Network was in order and there was no leakage found in G I Earthling Wire of Said LT Pole. Due to Leakage of potential in TV Cable Network through G I Wire of TV Cable Telephone Tubular Pole got electrocuted. At that time Cow passing
nearer to Telephone pole got electric shock and died, Source of leakage current are under investigation' After investigation, it will be finalized whether accident occurred in network or Private Premises.

As information received from local person of village Ramper-Beti, regarding this accident on dated. 21.08.2018 at 14:00 Hrs, immediately DE Kuvadva Sub-division reached the site and found that the victim went to terrace of house for sleep, during this victim tried to stroke the wet blanket, at that time blanket wrapped to "R" Phase of wire of 11 KV line, the victim got electric shock and admitted to CHC Kuvadava, Where doctor declared him dead after some preliminary treatment. So due to tripping in 11 KV Samay feeder reflected current / jerk passes through earthing in DP of 11 KV Ravki Ag feeder, on which both victims were working. Therefore, both victim got minor current / jerk. Immediately both victims referred at hospital for treatment & after preliminary treatment both victims were discharged from hospital as minor electric shock and no burns spot injuries to both of them. At approximately 11:50am deshi organic fertilizer was laid in the field by tractor trolley by labour Sh Shantilalbhai Narayanbhai and other 4 labours. Somehow unknowingly tractor driver raised hydraulic trolley below above mention 11kv AG feeder span and that raised trolley accidentally touched 11kv Anjani feeder line. At this time labour Sh Shantilal was pulling fertilizer from tractor trolley by a khapali with steel handle & he got electrocuted and died. However, detailed investigation will soon follows.

On Dt:-24.07.2018 LT AB cable burnt & broken, which is removed from PSC pole to ground for binding purpose, which is jointed & binding of broken AB cable Victim climb on LT pole for reinstalling, at the time of climb on pole approx. 10 feet. Due to very old pole age of about 27 year old Dt:-5.12.1991, LT pole broken inside from ground surface & PSC pole fall slowly towards ground, at a same time victim jump from LT pole to ground surface. As victim wear safety gadgets like helmet, safety shoes etc., hence no any major injury occur & met with non-fatal mechanical accident. This PSC pole already rusted below ground level which is not visible. Then victim immediately admitted at Krishna Hospital Morbi & after approx. 1 hour victim discharge from Hospital due to no any major injury to victim & then on next day Dt:-25.07.2018 victim face stomach pain & admitted at Rajkot sterling hospital for further treatment, his health is good No any serious injury. As per site visit & prima facie investigation that victim and his co-worker went for installation of DT Meter on AG TC of Alibhai Haiyatbhai Kadivar under 11 KV Moriya Ag Feeder It has came to know that two no's of DO Fuses were removed totally by them and the 3'd Do Fuse found blown off and hanged on lower part of Do. Sparking black spots are found on HT side stud and Breather Pipe of said TC. So it is concluded that 3rd DO fuse not removed by them and during work victim came in contact with HT side stud of TC and got electric shock and fell down on ground. Co-worker and villagers has admitted victim to Civil Hospital Wankaner immediately, where concern medical officer has declared him dead. Burn spots are observed on left hand and abdomen of victim’s body. More over there is no any tripping recorded in 66 KV Jadeshwar SS at that time. It has come to know that hand gloves and other safety gadgets were not used by them. So it seems that accident occurred due to negligence of victim and co-worker. They were working without supervisor. However, detail investigation report follows.

As per site visit, it was found that at the place of accident, Accident took place at 11KV Thorala AG feeder HT pole no MOB/THR/058/R-39. On that pole we have found sparking spots at two places one is at just above V-cross arm & second at approx. 4-feet below the V-cross arm. And also we have found sparking spots at 11KV conductor approx. one feet away from pin point. So by making investigation from his relatives, they said that he was mentally weak hence it can be concluded that, due to his mental dis ability, Victim was claimed at 11KV pole and touched live 11KV conductor by his hand & done suicide attempt knowing that line is live. Hence victim got electrocuted and got fatal accident. As per the information from 66KV Morbi-B S/S on Dtd-01.09.18 a tripping was found at 15:35 Hrs. on 11KV Thorala AG feeder. They have done one installation of TC in FCI Ag feeder and gone to other location. Without doing line short and earthing works as per safety norms and time care not taken to confirm which feeder for the other 2nd SPA work in Dared Ag or FCI & directly work carried out on other location which come under Dared Ag Feeder. They hoisted the transformer and completed one phase jumper and during the work on second jumper the electric supply on dared feeder resumed. This feeder is in ON condition & no LC taken for above work. Hence, the contractor labour came in direct contact with 11 KV line thus one person got electrocuted & fatal accident took place & other person got fractured & admitted to hospital.

In Pragpar Ag Feeder fault, after line patrolling team of LM, EA found that the opposite of Rudrani Hotel at Jindal labour colony road on double pole structure the cable that is charged and its outdoor box it burnt. They decided to separate this fault cable and charge the spare cable that is already physically hanged on DP structure. After line clear taken and section was separated from load and main side section was physically earthed by Earth road and earthing chain. After completion of work separating of old faulty cable and connect line with spare cable, Indrasinh S. Baria was started to descending from DP structure he removed hook of safety belt from side clamp that was attached with V-cross armed his body suddenly got his body unbalanced and he slipped and his pent entangled with support angle of cable and he fallen on the rocky land at site. He moved to Jindal colony hospital for primary treatment and then he moved to mundra hospital for other treatment. At present he shifted to Sterling Hospital Gandhidham for next treatment. At site visit we found that the section where the work was carried out, it was sectionalized and earthed at both the end of work placed. Victim had used safety shoes, safety hand gloves , helmet and safety belt to complete the work. Victim fallen from the height of approx. 8 Mtrs. on rocky land at the time of work completed and started to descending DP structure. This was Mechanical accident occurred.

To give power supply affected area due to failure of HT ABC, power supply was given from 11 klv Jakhdada AG feeder by giving temporary jumpers- On date : 24.09.18, replacement of failed HT ABC by 55mm2 conductor was arranged with the labour gang of contractor Shri Vipul Sorathiya. Initially when the work was started, the location from where power of 11 KV Jakhdada feeder carne towards location of replacement of HT ABC, all conductors of one Span were temporarily removed. So that power cannot came to the location where work to be carried out. When the replacement work to be nearly ending, labour namely Shri Madusingh Babusingh Rawat was went to the location, without informing to site supervisor as well to contractor & without instruction of all above where conductor of one span was removed. At that location he had taken one of remove conductor & climbed on the pole to join the conductor. This work is to be carried at the end & after taking LC of 11 KV Jakhdada feeder. As on that pole, there was power of 11 KV Jakhdada feeder, as he was climbed on that pole & swipe his hand reversely. He got electric shock & through the conductor which with he climbed the pole, power came in one of phase of isolated line & hence, 2 other labours who were working also get electric shock. Thus, the accident was occurred due to wrong action of labour Shri Madusibgh Babusingh Rawat & also he was not instructed to do so. Site supervisor is Junior Engineer Shri Ashish Mothalia. Two labours namely 1) Madusigh Balusingh Rawat & 2) Pappusingh Balusingh Rawat was admitted to Lilawati Hospital for treatment. Both above labours are in good health & out of danger. Third labour namely Ladusigh Omsingh Rawat was admitted to Anjar Civil Hospital & from there he was referred to Bhuj civil hospital for further treatment. At present his health is under improvement & he is kept in under observation for 48 hrs.

As per demand of Shri Kalasva, Gamit received L.C.P. no-1447 Dt.07.06.2018,at 11:40 a.m. and then informed to Shri kalasva and asked to work after making line short and earthed, but without checking line is live or not at consumer installation or removing 11KV D.O. fuse link, he climbed on TC structure and got electrocuted and fallen down on earth. He got minor head and back side injury and admitted to private Rachana Hospital, Botad for indoor treatment. Actually consumer was getting power supply of 11KV Ningala AG feeder but it seems misunderstanding in taking line clear of feeder due to which the said accident is occurred.

After completion of work, line clear was returned and Shri B J Dafda and Contractor gang were departed from that site. At the same time Victim Sh.B M Makwana also reached at site as he was resident of same village. B.J.Dafda has also informed him about Power was ON. After power ON, there was still a problem in TC. Hence as Victim was present at site, he has done line patrolling and found jumper fault in taping line by him. After finding fault, he had informed to Sh.V.A.Dhadhal (EA) village helper of that village and told to take line clear of Rameshwar Ag feeder, as Victim was usually working with Sh.V.A.Dadhal in his allotted villages. Sh.V.A.Dhadhal has asked for hand Tripp instead of Line clear from GETCO and after getting confirmation he had told to Victim about Power OFF as well as also told him to start work only after he reached there. But Victim himself resident of that village ignoring the instructions in hurry and over confidence tried to repair Jumper Fault on the live line and got electric shock & met with Fatal Accident because as per information received GETCO operator has Hand Tripp 11kv Virpur JGY instead of 11kv Rameshwar Ag through mistake and hence power was already ON in feeder where work was to be carried out.

After electrical shock, it is informed that she fell down from the tree on the ground with soft soil and so, did not got any physical damage or bruises. Since she got burns on her 3 fingers and palm and leakage current passed from her body and left from toes and front side of her foots, it is suspected that while she might have tried to overstretch herself by standing on toes, she got shocked and burns on her fingers and toes. Today, while visiting victims’ house, details was told by her in her own words and confessed that she got shocked while she was standing in the tree at some level and while trying to stretch out to pick up, she got shocked and fell down. Except burns in fingers and toes, she appears to be in good health at present.

There was on accident day said buffalo rubbing it’s head with electric pole. On that pole street light connection of Gram Panchayat exist. In that street light connection one phase dis connected and touch with earth wire of that pole. So current passes through earth wire and same time buffalo touch to earth wire and buffalo got electrocuted.

A fatal electrical accident occurred to khimjibhai bhimjibhai mithapara while cutting tree branches at prabhubhai dharmshibhai zarmariya’s farm. Neither prabhubhai dharmashibhai zarmariya nor khimjibhai bhimjibhai mithapara cut a tree from bottom and tree fall down on 11 kv chokdi ag feeder but tree was wet due to rain before few hour of the accident. Hence leakage current passed from 11 kv chokadi ag feeder line to tree to khimjibhai’s body and khimjibhai bhimjibhai mithapara got electrocuted.

During inspection of site, it was came to know that on date 30.08.2018 two cousin sisters were going in Ag land farm of Visnagar village for playing at approximately time 12:30 pm (30.08.18),from their houses after eating lunch. But they were not come at their houses about 3:30 pm (30.08.18). So their parents and neighbours went for finding both cousin sisters in villages and nearby Ag land areas. At approx. time 12:20 AM on date 31.08.2018 both sisters (victims) were found on the Varakhadi tree in situation of laid down in fatal condition of ag farm of Harjibhai Arajanbhai Dabhi. Varakhadi tree’s top branches far away in horizontal distance approx. 2feets.from nearby conductor and 1 to 1.5 feet above of 11 KV Rupasan AG feeder emanating from 66 KV Dhama ss.it might be due to weight of victims during playing climbed on the Varakhadi tree and one top branch tilted and touched to live nearby conductor and current passing away from tree branches of Varakhadi tree to victims’ bodies and both victims were electrocuted. Three phase power from 01:45 pm (30.08.18)&first Tripping was observed at 7:00 pm(30.08.2018),try taken on19:05pm(30.08.18) and feeder put in PF& light rain continue in this area &then 9:00 pm (30.08.18) again try give &feeder charged. Again feeder goes in PF at 11:05 pm(30.08.18) which charged at 2:30pm(31.08.18) today by ground patrolling by line staff and bird fault and jumper burnt found at Rozva tap line..Sarpanch of Visnagar village has informed to operator of SS at 01:00 AM (31.08.18) And operator has informed to DE Dasada at 01:15 am(31.08.18) also shock sign observed on both victims hand & leg.

During inspection of site it was came to known that on date.23.09.2018 in the morning the crowd of buffalos were gathered nearby open place at water work transformer at Adariyana village. Among theme three buffalos (victims) were going near to water work transformer center where water logging from Sump of water works connection. At that time one Crow (Bird) was sitting on the Top plate of the above water works transformer and touched to two LT PHASE STUD of above transformer. Due to that LT SIDE TWO PHASE Sort and transformer made body earthed and current passed from transformer to TC centre with Angle and same time one big buffalo came to above near TC Structure and Rub that body with PSC pole and touched clamp of supporting angle. due to this leakage current was passed from transformer body to transformer structure with clamp of supporting angle to body of that big buffalo and other two buffalos came in direct contact to this sort buffalo and all above three buffalos electrocuted and laid down on water logged land area near above transformer center. Earthing pipe was existing and no leakage current found after fault clear.
