3 RCC Navagam-Mukeshbhai Mohanbhai Dudhat  13.10.16 FHOS "This incident came to known from Kuvadava Road Police Station. On site visit  and eye witness it concluded that the victim while supervising the construction, which is under the 11 KV line, lost his balance and accidentally came in contact with 11 KV line and fall down on ground & had head injury met with non-fatal accident. He was admitted to Sterling Hospital, Rajkot, where he died on 28.10.2016 thus non-fatal accident converted to fatal accident.           

The victim him self had applied for line shifting and has paid line shifting charge on 08.10.2016 & was inform  in estimate to do any construction work near or beneath the electric line after it is shifted . 4 RCC Rajkot-Shravannag Shukarunag 15.03.16 FHOS As per site situation / site Rojkam / co-worker statement / statement of owner of premises / details regarding tripping from these supporting evidences it is not clear that the accident occurred due to electric shock. But, as per PM report of victim, it is mentioned that reason for death “Shock due to electricity current burn”. 6 RCC Rajkot-Pruthvi Rameshbhai Sarvaiya 08.01.17 FHOS A victim trying to catch kite from 11 KV line by using approximately 10 ft long iron rod from his terrace. Hence when iron rod came in contact with 11 KV line, victim electrocuted due to current passes through iron rod. 8 RCC Rajkot-Hitesh Girdharbhai Parmar 23.02.17 FHOS About accident come to know from police station. As per site visit and panch rojkam it concluded that victim– labour of   contractor Shri. Jitendrabhai Harjivanbhai Nadiyapara doing construction work on terrace of bathroom on first floor of house of Poonam Devlopers. While doing so, levelling work with the help of 5.5 ft aluminium section strip, this strip come in induction zone of conductor of 11 KV Alap feeder which was about 2 feet away from the wall of new construction works and got electric shock and fell down on ground level and was admitted to Giriraj Hospital and then to civil hospital where he declare dead thus met with fatal accident. Inspecting aluminium strip no black spot found on aluminium strip nor on wire of 11 KV. No tripping observed on 11 KV Alap feeder at time accident.
For construction work temporary connection no. 986 obtain from Nana Mava s/dn. This accident occurred due to irregular construction without keeping safe distance from the existing 11 KV line conductor. 9 RCC Rajkot-Dhirubhai Keshubhai Jotaniya 09.03.17 FHOS About accident come to know from police department on dt.10.03.17.from site visit it found that victim while doing construction on first floor. For this he was lifting iron rod("khilasali")of about 12 ft long from ground floor to first floor while that tail end of iron rod come in touch with live conductor of 11 KV Omnagar feeder and got electric shock and met with fatal accident. Horizontal distance of conductor from the wall is 9 feet. Tripping was observed on 11 KV Omnagar feeder at time of accident. 10 RRC Kotda Sangani         Girish Devsi Rathod 03.10.16 FHOS Intimation received from  police department on dated 13/10/2016 and site is visited on date 14/10/2016. As per victim’s father statement taken and observations during site visit we found that accident occurred due to illegal additional   construction of residential premise near 11 kv Kotda JGY FDR.  The owner of house is  Shree Devshibhai Bhimabhai Rathod(victim’s father ). The victim was standing on the terrace   of this single storey premises and while he was splashing water on newly constructed parapet wall and other new construction with the help of tumbler by hand and came in induction zone of 11 kv feeder. So got electrocuted. Further investigation is under progess. Victim himself is responsible for said accident. 11 RRC Jam Kandorna -2         Rajendrasinh Ratansinh Rajput(Contractor Person) 08.10.16 FHOS This fatal accident occurred to the above person(victim) of contractor M/s Sati Aai Krupa enterprise, when he had climbed to a live 11 kv Dadar ag line pole near road cross before completing of L C procedure.
A new JGY link line was being erected for new 66 kv Gundasari s/s .This new JGY link line route is crossing over the existing road cross of 11 kv Dadar AG feeder. To make crossing of these two lines, pole are erected on both sides of existing 11 kv Dadar ag feeder road cross. To carry out this new JGY link line work, two line clear permits had been asked for 11 kv Baradiya ag feeder ( at 3:28:16 pm) and for 11 kv Dadar ag feeder ( at 3:33:30 pm) by the PGVCL's supervisor Shri V.J.Bhokiya, EA (VS) by SMS who was present at site. Mr. Bhokiya had received an SMS from 66 kv Chitravd s/s for the LC no 6431 for 11 l(/ Baradiya ag feeder (at 3:35:30pm),but the SMS of LC of 1'1 kv Dadar ag feeder was pending to be received. Mean while, at that time, the victim climbed to the pole without taking confirmation of both LCs, without observing safety rules (viz line discharge, earthing and making short of 3 phases) and without using safety gadgets (viz safety belt& helmet) and touched the tive wire of 11 kv Dadar AG feeder and spark occurred and fall down on ground.( A TT in Earth fault relay is recorded on 11 Kv DadarAg feeder panel at 15:30 hrs, SMS for LC of 11 KV DadarAg feeder received at 3:48:07pm).lmmediately, the victim was taken to the Jamkandorna civil hospital and then after Dhoraji civil hospital where the probable cause of death is found due to cardio-respiratory arrest due to ele$rocution. pM report is awaited.

12 RRC Navagadh          Soniya(Sunil) Nanajibhai Rathava(Alm) 16.10.16 NFHD    Accident occurred to the victim when the victim with LM Sh. Nanjibhai Sutariya went to attend a fault of a W/W TC lug of 11KV side Y phase. The TC is on 11KV Pithdia JGY feeder. They have taken LC of 11 KV Jepur JGY feeder instead of 11 KV Pithdia JGY feeder. The victim climbed on TC pole from LT side wearing hand gloves and came in contact of LT and induction zone of live 11 KV line. Immediately he was taken to Hospital at Navagadh and then after to Sterling Hospital Rajkot. 13 RRC Jam Kandorna -1           Babubhai Z. Katara (Ea) 02.11.16 NFHD Accident occurred to the victim when the victim with
shri. H D Makwana, L.M.went to attend a fault of neutral missing on AG connection of Madhubhai
Nagjibhai Valu TC.
They have cut the D,O. fuse element of two phases and remaining third phase D.O. fuse
element was remained unremoved with hanging position from upper side of live 11 k.v. line with
gap of approx 2 inch from lower side Gontact. The victim climbed on T.C. pole from H.T. side
without using safety gadgets and touch'the jumper connecting DT phase and D.O. lower contact,
thus indirectly came in contact of induction zone of live 11K.V. line. Immediately he was taken to
hospital at jamkandorna and then after to Dr.Chikhaliiya's Hospital at junagadh. The matter is
under further investigation. 14 RRC Vichhiya      Vishlbhai Pravinbhai Rangpar 11.11.16 FHOS As information got from police station on DT: 15.11.2016,DE-Vinchhiya rushed at site, as per eye witness statement Main service of the AG connection of Savshibhai Popatbhai Rangapara (con. No. 81821003373) is burned and fallen down on the earth at that time Victim was passing there and he came in contact with burned service and he got electric shock. In this connection no any leakage current found in PGVCL network and in load side wiring protective device like MCB/ELCB is not provided. Burnt service is taken under custody for forensic Lab Examination. Further investigation is under progress. 15 RRC Vichhiya         Sanjaybhai Ranchhodbhai Taviya 14.11.16 FHOS As per site visit and statement of eye witness, victim Sh. Sanjaybhai Ranchhodbhai Taviya was doing unauthorized work at nearby 10KVA Transformer of Sh. Devabhai Rupabhai Taviya (con.no:81820004937),At that time he came in contact with live 11KV Bushing and got electric shock. 16 RRC Lodhika      Kamleshbhai Mangalbhai Patel (Contractor Person) 29.11.16 NFHOS On dated 29.11.2016 at approximately 4:00 PM contractor’s labour(outsider) Shri Kamleshbhai Mangalbhai Patel was working for HVDS work  Of 11 KV AG Dom feeder at village Kalyanpur Ta. Lodhika, Dist. Rajkot. In order to fixing tapping angle on existing transformer centre while climbing  on DP  of 11 KV Kangshiyali AG feeder instead of 11 KV Vagudad AG Feeder at village Kalyanpur Ta: lodhika DIs: Rajkot Accidently victim came in contact with live conductor of 11 KV Kangshiyali AG feeder and consequently got electric shock on his right hand. 17 RRC Metoda      Yogesh Kanubhai Dabhi 11.12.16 NFHOS On dated 19.12.2016 letter from metoda police choki (Inward No-2022 dt 19.12.2016) a non fatal accident occurred to Shri Yogesh Kanubhai Dabhi at Metoda GIDC at time approx 21:35.The consent SDO rushed to accident site for investigation,it is found that at accident place there is passing of  11 KV Kohinoor GIDC feeder from 66 KV central SS and in 11 KV line span between pole No   CNT/KOH/054/L-04 and DP 63 KVA black spot appears in two phases which is clearly shown from ground,also thread of kite wound from live wire to TC DO. There is possibility of this accident occurred due to victim was removing kite which is wound on wire by any conductive apparatuses and contact with indirectly with live wire and got electric shock at that time and victim was admitted to hospital for further treatment. Actual reason of accident will be known after reciving of medical report from hospital. 18 RRC Tramba    Mukeshbhai Ranchhodbhai Makwana 07.02.17 FHOS This Accident was occurred on the date 07.02.2017 and  DE-Tramba has visited site and as per accident place shown a victim has unauthorized filch guy wire from the ground and climb on the pole for removing guy wire that time upper v-cross arm is tilted and touched with live 11 kv conductor of Dhadhani Ag feeder and this accident occurred. 20 RRC Atkot       Narendarsinh Ramdayalsih Chauhan 18.02.17 FHOS Information regarding this accident was got From line staff and DE Atkot with DE tech Jasdan DO  has visited site on the date 18.02.2017(same day). As per site visit victim was doing jumper of HVDS work in Bakhalvad Ag feeder that time suddenly power started and victim got electric shock.
 After primary investigation LC of Bakhalvad Ag feeder was already taken by K.T. Bhesjadiya L.I. but nearer one feeder Dungarpur Ag feeder is “ON” it is also came to notice that there is one consumer who has two Ag connections one connection is given from Dungarpur Ag Feeder and another is given from Bakhalvad Ag feeder and this consumer is using both feeders power through one 3 core cable of approx 250 Mtr. long. This consumer has made one junction in this cable near his home at near Dungarpur Ag connection. Thorough this junction LT power of Dungarpur Feeder Fed to another connection of Bakhalvad feeder. Hence it may be possible that LT power of Dungarpur feeder become 11 KV power through transformer of Bakhalvad feeder and this accident occurred. Both connections are disconnected and both meters are remove for taking MRI and further investigation.

21 RRC Kotda Sangani       Dineshbhai Jagabhai Solanki 04.02.17 FHOS The guy foundation at above location was damaged by R&B department’s Highway Road construction agency KETAN CONSTRUCTION during construction work of under construction new bridge over Ghoghavadhar to Ramod highway road. So, when victim came into contact of above guy, guy rod came out from the ground and guy wire below guy insulator touched to jumper of tapping line on nearby PSC pole. So, the leakage power flown through the guy and victim. So, he electrocuted and died. This accident occurred mainly due to removal of guy rod by ketan construction. So, the procedure for lodging FIR against the same is initiated which is under process. 22 RRC Atkot       Jasmat Jadav Rojasara 31.01.17 FHOS WE got information regarding this accident date 14.02.17 by victim’s son Shri – Bhupatbhai Jadavbhai Rojasara’s letter. Accident was occurred on date 31.01.17, during treatment in hospitals victim died on date 05.02.17. And this Non fatal accident is converted in to fatal. As per Bhupatbhai statement Victim got injury like electric burns on his stomach, Chest, Legs and wrist of left hand.
 But During Site Visit of DE Atkot on date 15.02.2017 there is  11KV Jumper on TC from DO fuse in  Y Phase  11KV Bushing  have sparking , blackish burning signs. Also there is  not possible to victim was came in contact or touch  with  jumper of DO fuse (Busing to DO) hence it is not clear that this accident exactly which place occurred Hence, all three jumper of DO remove with Videography and photography and send for investigation in Forensic lab Rajkot.

25 RRC Upleta Pravin Babuji Rathod 19.03.17 FHOS This accident occurred at Supedi Village agricultural extension (khetivadi sim Vistar) in 11kv Zanzmer AG feeder on dt 19.03.17 under Upleta Rural Sdn. From the site investigation it is found that Victims were in truck (GTY-5166). Truck was loaded with agricultural waste (Coriander waste). This loaded truck passing parallel to 11kv line of Zanzmer AG feeder and extended metal projection of the truck came in to contact with live conductor. 52 JMN Jamnagar-Dipakbhai Veljibhai Nakum 03.12.16 FHD Victim was working on 11 KV crystal feeder on DP with DT number SRS/CRYSTAL FDR/011/L-018. Victim started his work with necessary safety tools, 3 phase of 11 KV side are shorted with short link (copper wire) and earthed with discharge rod on southside of DP structure, northside of 11 KV side not shorted & not earthed by lineman. Also DO fuse of all 3 phase of HT side not removed by lineman on the d p structure with transformer but 11 KV GO switch cut off, on which accident occurred. 11 KV crystal feeder LCP number 6651/03.12.2016 taken by B N Kotad (LM). Electric current of any generator may have been momentarily passed through line and passed through the body of victim and fatal accident occurred with him. As per CEI accident occurred due to improper earthing of all 3phase and not wearing of all safety gadgets by the victim specially safety shoes and gloves.  Accident can be prevented by proper earthing, use of safety gadgets and proper testing of line before starting of work. All staff were educated in safety meeting by SDO to carry out proper earthing and use of all safety gadgets for accident prevention. Strictly instruction given to all line staff to use safety measures & tools while working on line by weekly safety meeting. 6 As per CEI accident occurred due to improper earthing of all 3phase and not wearing of all safety gadgets by the victim specially safety shoes and gloves.  Accident can be prevented by proper earthing, use of safety gadgets and proper testing of line before starting of work. 53 JMN Bamangam-Kiritbhai Dayabhai Thakar 05.12.16 FHOS (Vijalighar helper) While victim has asked wrong feeder LC from Elec. Asst. and he climbed on DTC for service repairing work at Bhikhu Samji's farm and touched 11-KV Napaniya Khijadiya feeder and got electrocuted. 124 SNR Muli, Shri. Dudabhai Galabhai Zala 23.01.17 FHOS On date 24-01-2017 at approximately 9:00 Hrs. JE Shri M D PRAJAPATI was informed from police station that fatal electrical accident has occurred to Shri. Gugabhai Galabhai Zala No any person from the village or relatives of the deceased informed MULI PGVCL regarding the accident. The person was doing to take unauthorized single phase supply from 11 KV Mandavray AG fdr and that time Mandvaray AG fdr was in three phase supply and the person was hooking with that 11 KV line to capture power from line for use of unauthorized power supply and he was contact with the 11 KV line of hooking wire and got electric shock and immediately death occurs.
