From site visit it concluded that accident took place while victim was working on bonnet of truck which was parked under the 11 KV HT line and as soon as he stood up on truck bonnet come in induction zone of live wire and got electric shock and fell down from truck bonnet   and had injury on back side of head and met with fatal accident.  On investigation no tripping observed on 11 KV HT line side.

About  the accident come to know  by D.E. Vavdi  urban S/dn on date 26-04-16 so immediately site visit done by him in present of  Electrical inspector .From site visit and statement of victim’s brother it concluded that accident occurred on date 21-04-2016 while victim was doing some work unauthorisedly on LT line pole . During that  he fell down from LT line pole and admitted to civil hospital  and met with fatal accident ,As per statement of police and victim’s elder brother victim had bums on his finger . P.M. report of victim is awaited.

Anilbhai Jivrambhai Dhorecha came to the above address for carpentry work during night hours for measurement when he entered in the office at the first floor with 15 feet aluminum rod with his hand  Victim was trying to enter in the office along with this aluminum road he was cross the aluminum rod and it touched to the bare 11 KV live electric line of Royal Industrial feeder which is 9 feet away from accident place and victim got electric shock and died at site.

This accident occurred at 66kv Mandlikpur Ss compound in 11kv Fareni AG feeder on dt 08.08.16 under Dhoraji Rural Sdn. From the site investigation it is found that Victim knows that 11kv Bateshwar AG feeder was in PF from dt 08.08.16 time 4:00am due to cable fault at panel. So Victim took LC of 11kv Bateshwar AG feeder Vide LC No 6774 Dt 08.08.16 time 9:00am by SMS for Jumpering work. Victim has carried out Jumpering work in 11kv Bateshwar AG feeder and he came to 66kv Mandlikpur Ss at time 9:50am for remove of cable jumper at first DP of 11kv Bateshwar feeder. But by mistake victim climbed on DP of 11kv Fareni AG feeder. So Victim fall down from DP but in 11kv Fareni AG feeder have not any tripping in 66kv Madlikpur Ss  around time 10:05am. So Nonfatal injury occurred to Victim’s spinal and he admit to Jetpur Govt. Hospital for primary treatment and then admit to the Sterling Hospital at Rajkot.     This is for your kind information please.

As Per The Statement of eye Witness Sh maheshbhia jivrajbhai dobariya ,  Sh R L SarvaiyaBhai Come To his Ag Land To  Attend LT  Power Complaint Of Conn No-34859/05000/0 ,He Went to direct TC Centre & Only Cut Off D O Fuse than climbed on the TC And Repair The TC  Ckt Cable .After That he Climbed On LT  pole and Found that Span is Loose, so he tried to pull LT Conductor For Stringing but as he pull the conductor, the conductor touched 11 KV Line Passes over the LT Line & got Electric Shocked ,and  as his both legs were crossed lock in LT Wire he faint on lt pole with head down to earth  During primary investigation it is found that he had only cut off DO Fuse , no 11 KV LC Permit Taken From S/S ,Also No Local Earthing Line Short Done By Him ,also while working on lt pole he didn’t use the safety helmet as well as safety hand gloves , He had made complete violation of safety rules

The accident was informed by Sh.Dineshbhai at Sardhar fault office on dt:30.8.16. Also Press note published in local newspaper on dt:1.9.16. As per statements, the victim was climbed on Tree situated in his Ag land to cut the tree branches which likely to touch 11KV line passing nearby it. In doing so, the cute tree branch touched the 11Kv line & got electric shock & fallen down from the tree. This iron wire laid from top of the girder to land and it miight be throughly passed live current due to above reason hence this live wire laid on ground so any time female horse might be touched and might got shock and this wire wounded it's body same might be got unfortunately shock (electrocution) and dead. There is no any lacuna of PGVCL work/Staff etc but street light fixture was short and Dis TV network wire comes on contact through girder pole and this accident unfortunately.

At same time, another cow came into contact with electrocuted cow. She also got electric shock and this way both cow electrocuted and died on the spot.

A ftal animal accident occurred due to service wire was burnt and live part of service wire came in contact with fabrication of earthing which was connected to earthing wire so leakage current passed through the earthing wire, due to water logging area near the pole cow is passing from the water logged and current passing through its body and electrocuted.

Insulation of AB cable was damaged and live part of AB cable came in ci=ontact with fbrication and leakage current passed through fabrication in GI wire and due to heavy rain in last night land surrounding guy was wet and buffalo pasing from this area and electrocuted.
