At LT pole No 021 piece of 1 feet (approx) of remaining portion of removed “lungar” wire was hanging and live conductor of this wire come in touch with guarding wire so leakage current pass to next pole through guarding wire. At this next pole through earthing wire leakage current passed to earth and surrounding land of the pole get electrically charged ,so as soon as the cow came in the vicinity of the pole get electric shock due to step voltage and met fatal accident.

On date 10-07-14 accident took place near HT line girder pole No 032 in which leakage current was found passing through it. Guarding to the HT pole provided continuous from previous ploes at which HT and LT line erected on comman poles. On investigation and by switching “OFF” power of near by T/c leakage current in HT pole stopped and then on switching “ON” power of the T/c leakage current not fount in HT pole .So thus it concluded due windy and rainy condition earth wire of HT-LT line common pole may touch live part of LT line live wire and caused momentary leakage current in earth wire and as 11 KV guarding fitted on this common pole leakage current passed in guarding wire and through it in HT line pole at which accident took place. So as soon as cow comes in vicinity of the pole got electric shock and met fatal accident.

On date 13-07-14 Accident took place near T/C center PUN/SUP/039/DT-31. As message of accident received by the fault center line staff and Deputy Engineer immediately visited the site. At site a cow found laying dead near transformer center. At time of visit it was found that no any leakage current passing through T/C earthing, pole or in any near by LT pole. It was rainy and windy atmosphere at time of accident. So it concluded that due to momentary touching of Phase wire with neutral wire leakage current may passed in T/C earthing. So as soon as Cow comes in vicinity of T/C may got electric shock and met fatal accident.

Victim went on terrace to catch pigeon on his neighbor’s terrace from his terrace. He jumped over from small parapet between his house and neighbor’s house. While crossing the small parapet he loosen his balance. Though no any protection wall on terrace of his neighbor, he caught live wire to avoid him self to fallen down from balcony to “Vokala”. Resultantly he caught 11 kV live wire situated 1 feet away from his balcony and approximately 4 feet away from his wall. Thus he got an electric shock and expired.

On Dt.- 12.5.14 There was heavy wind pressure. Due to this disc insulator got damaged and conductor was broken of 11 kv Mahadev JGY feeder of DT-06. Buffalow and a calf was fasten under the 11 kv line by owner.. When the conductor snapped it directly fallen down on the buffalow standing under the line and got an electric shock, At the same time calf which was fasten near the buffalow came in contact with snapped conductor and got an electric shocked and both died.

Afterwards police panchnama was carried out on new site under Atkot S.Dn area & DE Atkot visited site and taken statement of eyewitness and owner of sighted T/C.As per statement of eyewitness Sh.Dhirubhai Gela Chavda accident was occured due to victim came in contact with HT line on T/C while doing unauthorize work on T/C.But on site visit no avidence or symptoms of accident found on T/C , wire , structure or psc poles and our PGVCL network found healthy. Detail cause of accident is under investigation.

ONE OF THE VICTIM DIED AT THE SITE ITSELF AND OTHER HAS BEEN ADMITTED IN CIVIL HOSPITAL RAJKOT.FURTHER INVESTIGATION AND DOCUMENTATION UNDER PROCESS.

Power supplied and in earthing strip power was on. Hence this buffalo was came in contact with that strip and got electric shock and this accident occurred.

On dated 13-Sep-2014, at time approx. 14:03 Hrs an electrical accident has occurred to a boy at Kotda-Ardoi road, opposite to the farm of Pravinbhai Bhikhabhai Bagda, at Ardoi village.
A boy aged about 7-yrs-old due to any reason climbed on the tree from where 11kv Ardoi ADOM feeder is passing nearing to it. Accidentally, victim came in contact with 11 KV line live conductor and got electrical shock and met with non-fatal human electrical accident. There were two eyewitness named, (1)Dineshbhai Devabhai Bagda, (2)Ramjibhai Desha Bhai Bagda of ardoi village who were present at opposite location of accident site.
For further treatment victim was transferred at Government hospital Rajkot. On site, there were black spot found on the conductor of 11kv Ardoi ADOM feeder. A statement of eyewitness was taken and site Rojkam was done and also local police station was informed for above incident.

The victim is hired as a labourer by contractor undertaking PGVCL line work. The victim was into line maintenance work of 11KV Kashipar AG feeder emanating from 66KV Daldi S/S. The line clearance permit of the concerned feeder was duly taken by line inspector before commencing the maintenance work of this feeder. Further as a safety/ protective compliance, all 3 phases had been shorted & earthed before commencement of the aforesaid work. However, technician of the 66KV Daldi S/S switched ON the 11KV Kashipar feeder even though line clear permit was not returned. As reported by the SDO Lodhika;On date: 26.01.15 the 11 KV Pardi AG feeder was in Fault at 14:00 Hrs which was attended by LM of Lodhika SDN Sh.L.G.Jotangiya with EA Sh.Jitubhai Khuman. While working for rejumper at Substation end Sh.Jotangiya observed that inspite of 11 KV Pardi AG Feeder in PF, the power felt when discharging the line with the discharge rod and after opening the GO switch located at “Pardi Vokala” the return power not felt. Consequently line patrolling carried out but no return power location found hence approx. 19:00 Hrs the Pardi AG feeder made ON in single phasing with the intimation to Line Inspector Sh.M.S.Bhoye to find out the reasons on the next day i.e. on 27.01.15 after the completion of scheduled 3 ph power of 11 KV Pardi AG feeder which is 04:00 to 12:00 Hrs. On date:27.01.15 the Line clear of 11 KV Pardi AG DOM feeder has been taken by Sh. L.G.Jotangiya LM barring LC No:17173 time:12:30 Hrs. He and the other line staff viz. 1.Sh.Mohanbhai S Bhoye LI; 2.Sh.Gopalbhai L.Damor LM; 3.Victim Sh. M.P.Kandoliya ALM; 4.Sh.M.S.Miyatra EA; 5.Sh.R.J.Jadeja EA carried out the work of patrolling and to sectionalize the feeder for the detection of return power to 11 KV Pardi AG feeders. During the course of work Sh.Jotangiya and Sh.Miyatra isolated the Pardi Wokla wali switch and provided local earthing to one of the phase to guy wire but not short all three phase wires. They both stayed there and the other line staffs went for the sectionalization on the feeder. At the place of accident i.e. on the PSC Pole DP with the 11 KV Out door cable box near the Tejasavi Hanuman Mandir the victim Sh.M.P.Kandoliya;ALM and Sh.G.L.Damor;LM had climbed the PSC Pole DP from the opposite end while Sh. M.S Bhoye,LI and Sh.R.J.Jadeja EA were on ground for supervising and assist the work. The local earthing; short of all three barred wires and the discharge were not provided on either side of the DP. The victim while working with the safety gloves to remove the UG cable jumpers from line to isolate the section sitting on the DP angle; suddenly got the electric shock when LI Sh.Bhoye found that the victim on the DP got the electric shock; he started climbing the Pole for the help of the victim but the victim fell down from the DP. Immediately, they all together give him the preliminary treatment and called ‘108’ambulaence. During that time they shifted the victim in hired SDN vehicle and met with ‘108’ near 66 KV Vavdi SS took him to Civil Hospital-Rajkot where the doctor in charge declared him dead. After the accident occurred the under signed with DE Tech RRD immediately rushed to the accident place and with the Deputy Engineer, junior Engineers and the line staff of Lodhika sub division had worked to identify the cause of the return power to 11 KV Pardi AG feeder on which the victim working. As found the AG RL 1phase consumer Sh.Dineshbhai Hansrajbhai Mungpara Con No: 33421-66761-6 LS No:161p36 village: Pardi. The said connection is supplied by erecting the 1p2w line of LT AB cable from the 10 KVA transformer connected on 11 KV Pardi AG feeder erected for the AG connection of Sh.Vipul Popat Parsana Con No:33421-01950-9. Sh. Dineshbhai Hansrajbhai Mungpara had illegally taken the direct lungariya from the LT line pole of 100 KVA, Pardi village TC on 11 KV Velnath JGY feeder by lying approx.1000 MTR twin core wire. He had provided the arrangement of Kit kat fuses and change over switch in his house for the alternate supply. It is found that from the change over switch the live phase of velnath JGY feeder power returned on 11 KV Pardi AG DOM feeder through the 1ph meter and AG TC of 10 KVA Transformer. Immediately the said AG RL connection is disconnected and the meter, change over switch, MCB unit, wires and arrangement are seized. Necessary legal steps have initiated on the defaulter consumer. PM report and Police Panchanam awaited.

A complaint of AG consumer named Babulal Samji Bhanushali of village Hajapar was lodged on 3-1-2015. This connection was connected to 11KV Nanavada AG feeder from 66KV Hajapar S/S. Victim H.D. Jangam was Electrical Asistant (village helper) of Hajapar Village. So Shri H D Jangam had gone to attend the complaint where a LT conductor was broken. A prearranged shut down was scheduled of 11 KV Nanavada ag feeder, from 66KV Hajapar S/S to carry out maintenance of the line. Line clear permit was taken by ALM–shree P.K. Gadhavi at 9:45 AM on 3-1-2015.

As victim shri H D Jangam had to carry out complaint attending work in same feeder for which line clear was taken by Shri P K Gadhavi, victim had telephonic communication with ALM shree P.K. Gadhavi and after confirming the fact with Shri Gadhvi that line clear was taken, the victim started work of repairing of snapped conductor of LT line of 63 KVA –Parvatiben Monji ag group transformer. The transformer was connected on 11 KV Nanavada AG feeder. The victim had competed conductor joining work from one side of broken LT line conductor. While repairing (Joining) of other side of conductor, power was restored from 66 KV Hajapar SS in 11KV Nanavada AG feeder, even though line clear permit was NOT RETURNED by ALM Shree P.K. Gadgavi. The sub station staff had energized said feeder without returning of Line clear by Shri Gadhvi and ultimately victim was electrocuted on the spot.

As per detail investigation report, MRI data, Log book and statement of staff, it is clear that sub division staff had energized 11 KV Nanavada AG feeder without confirming or getting mandatory return of Line clear permit from Shri Gadhvi. Thus, Sub station staff is primarily responsible for this accident.

11 KV conductor was snapped due to tree branches thrown on 11 KV live line by some illiterate and unsocial element. The tree branch was fixed between straight -cross arm and conductor. Due to moisture an electric path occured between straight-cross arm and conductor, thus continuous sparking occured at conductor result in to a conductor snapped and fall down on animals passing under the line and got electrocuted.

While investigation it is found in Preliminary stage, the accident occurred at Dhrobana Kalodungar main road. The two sheep were grazing at farm near to road, which is near to the 11kv DHARAMSALA JGY FEEDER. At that time, all of sudden 11KV conductor Brocken near 11KV disc insulator and due to 11KV disc Insulator fault. Due to it conductor snapped and while falling on ground, the body parts of sheep came in contact with the Brocken conductor and got electrocuted and died on the spot at once. Brocken conductor has been found from this place and also noticed deep burnt injuries on both sheep at that place.

At village Alampar, near Bapa Sitaram Madhi, a DTC of 16 KVA is erected for W/w connection. There is sump near the w/w connection. Water supply of the sump is coming from the Kalatalav Water Head Pump. The wall of the sump is damaged and water is coming out of the sump. So there is a water clogging in surrounding area including the place of DTC. Due to clogging of water and also earthing was deteriorated and the earhting of the DTC was joined with guy wire by someone else. Due to leakage current of the earthing the guy wire and surround water was electrocuted. The cow of Shri Arjanbhai Bijalbhai Khatana came near the DTC to drink the water. It might get shock and died.
