Tha horizontal distance of rack from distribution transformer was approx 1.0 meter and While doing the work , viticm lost his balance and might have come in contact with the upper part stud of DO fuse. From the Press Note of Aajkal; Dtd. 22-04-09 about Non Fatal Accident. occurred to Shri Vinubhai Savdasbhai Vasoya due to touching the Girder Pole near Aji Dam Checkdi, Deputy Engineer Aji Ind Subdivision has visited the site on dated 23-04-09 about 9.00 A.M. It is found that there is no any conductor of LT Line. There is cable with PVC Sintex Box and hence there is no any chance of leakage of power and no evidance also found for leakage of power.No any Complaint regarding leakage of power is registered in Aji Ind Subdivision.Victim was admitted to Hospital and he was not in a position to produce his statement. So, Statement of Victim is not taken.

On dtd.26.05.2009 , Mass maintainance programme was arranged on 11 KV GHB feeder under Aji-1 S/dn . For this Mass maintainance work , various employees were called upon from other s/Dn. Accordaingly ,victim was called upon from Aji-2 S/dn. to carry out work of Aji-1 S/dn..The victim was working on particular PSC pole of National High Way Road crossing span. 11 KV GHB feeder is crossing NH 8A. There exists PSC pole shacle point near by that particular pole having V-CROSS ARM on which vicitm was working. The Victim was doing work of replacement of 11 KV Pin by 22 KV Pin. The BInding of Conductor was removed and as a result of which conductor became loose which ultimately resulted into loosing of High Way crossing span. Due to reduction of distance of conductor from land, one Truck passing on Road came in contact with loose span conductor as result conductor got snapped and due to which PSC pole of shckle point was broken. Although victim was working PSC pole having V CROSS arm pin point. He got afraid due to snapping of conductor and breaking of PSC pole. Unfortunately , victim got injury (facture in both legs)and he was admitted to Dhakan Hospital where required surgery was carried out. This was purely mechanical Accident.

On Dtd: 24.6.2009 at Approximately 08:00 hrs., a Cow was passing along the street. At the time of accident,a 1 Ph. RL Conn. of Shri. Haribhai Talshibhai Makvana,having twisted with G.I. Wire and Breaking of Service Wire, due to this opening of insulation of service wire and touch with supporting G.I. wire and also same G.I. Wire touch with RMC Street light Fixture which is fix on same LT Line PSC Pole and hence current passing from G.I. Wire to PSC Pole through RMC Street light Fixture.Due to continue rain, a PSC Pole got wet and leakage current passing from PSC Pole to earth. At the time a cow passing near to PSC Pole and came in contact with leakage current and got the electric shock and died on the spot.

A Victim was sit inside the crane. While Crane was moving towards the LT Line, the upper part (A lifting part) of the crane was touched with LT Line. On Dtd: 18.7.2009 at Approximately 10:15 hrs., a Cow was passing along the street & Passing Near to the Transformer Center PUN/VID/DT37/5288, a Leakage Current passing in Earthing G.I. Wire of Transformer Center. After LT Line Petroling Pole to Pole, at LT Line PSC Pole No. VID/DT37/V-5817 a Private Tube-light Fixing of this Pole and 5 Amp. Switch with Flexible PVC Wire is also connected to B-Phase of LT Line Wire to this pole. A Phase of Flexible Wire touch with Guarding Wire and insulation of Flexible Wire removed and hence current passing through Guarding wire to Transformer Center. A cow touch this Earthing Wire at Transformer Center and came in contact with leakage current and got the electric shock and died on the spot.

Accident occurred due to leakage power
flowing from the vacuum motor of the Industrial Consumer’s Premises. The load side wiring was untidy, open & loose. Also, the vacuum motor was located in the court yard without ceiling & roof. The motor body earthing was not present. The motor plug switch was “OFF”. But, as the motor plug switch was defective so, the leakage power was existing irrespective of “OFF” position of the switch. Due to some internal fault of the motor, this leakage power was there in the motor body. So, when victim touched the motor, got electric shock.

11 KV Fdr. Bifurcation work of Aji Vasahat feeder was in progress, work of T/C. Shifting was carried-out by contractor's staff. DT No.10 was to be shifted under the new errected line. DP and S/S set was created under that line. Work of installation of projection set was carried out during accident. Two man were working in upper part and victim was Standing on DP and bridging in work with upper part and ground part, suddenly victim lost his balance and bring down to earth from DP. At the same time Angle falled down by either man working at upprer part of line by mistake and Angle was struck to the left hand of the victim and injured. Thus Mechanical Accident took place.
