The top conductor of 11KV Mahendranagar Ind Feeder snapped from between Location# Bharatnagar/M’nagar/072 (Pin point 11Mtr Girder Pole) & Location# Bharatnagar/M’nagar/073 (Transformer on 11Mtr Girder Pole). The snapped conductor then fell on the victim standing immediately below it & thus the victim met with the non-fatal accident. Primary analysis reveals that the technical reason for this snapped conductor could be the kite that entangled on the conductor of the said feeder between location# 82 & location#83, which then momentarily may have shorted 2 conductors.

On PSC pole# 145L009 of 11KV Amarsar AG Feeder, the pin fault resulted into leakage current onto PSC pole, its fabrication & the guy wire. These 3 buffaloes were drinking water from nearby water logged area which was about 10FT away from this PSC pole. The land surface immediately on & around PSC pole was dry & the wet land was about 10 FT away. The leakage current, created due to pin-fault, may have passed onto wet land where these buffaloes where drinking water either through Babul tree’s green branch touching the guy wire above guy insulator or through the dry earth surface area below PSC pole. As there were sparking/ black marks on the branch of the tree which touched the guy wire & also over iron rods of PSC pole & that this babul tree’s roots sprayed across wet land, the first theory that the leakage current passed onto wet land through green branch seems most probable.

Due to this construction structure below line, the vertical clearance of the conductor reduced to 6.8FT. However from ground the vertical clearance is 20.1FT. The victim while working over the slab of this under construction structure may have come in contact with the conductor through wet wooden plank.

The victim buffalo out of fright frantically ran & collided with the guy wire. The guy wire uprooted from the ground due to sheer force of the collision of the victim animal. The guy wire was of 11MTR girder pole through which highway crossing was given. The uprooted guy wire touched live wire touched live conductor of the same 11MTR girder pole. The conductor contacting the guy wire was tied to this girder pole at normal PSC pole line height & this conductor was then raised by providing jumpers to the top of this 11MTR girder pole so that necessary clearance could be maintained for Highway crossing. The guy wire thus became live as the guy insulator was provided between the heights of these 2 levelled conductors. The buffalo in contact with this guy wire thus electrocuted.

The victim was plucking jujube fruits from his farm’s tree using galvanised pipe of about 20FT, which had a hook like structure wielded at its one end. The victim lost balance or may have slipped as he stood over steep slope-like ground surface & the galvanised pipe tilted & fell over near-by passing bare conductor of AG feeder, which was about 4FT away from this tree. The hook like structure at the end of the galvanised pipe stuck & tangled with the conductor. The victim in contact with this pipe thus met with non-fatal accident.

We received a call from 8511181076 number at 12:45 pm on Dt : 10.3.2016 regarding one fatal accident at Modern Talkies near Chhathibari Road. As per his information the accident was occurred at 8:30 am in the morning. The victim was working in the construction building and when he lift up the iron rod , the rod touched the line and he got electrocuted. When we reach the site for investigation we asked in the Sub Station about the 11KV Chhathibari Feeder Tripping. But there was no Tripping in the Feeder. So it is not clear that how he got electrocuted. We asked in the Bhuj City 1 Line office also about any call for accident. But no one had called for any accident. It is also clear that the accident has been occurred in the private premises. We have also taken the snaps of the site. The further investigation shall be taken after PM report from Hospital only.

So as per site rojkam made by Jr Engg Kalawad road S/dn and HT -3 S/dn it concluded that accident occurred to victim looks mental retarded person as informed by eye witness .As per eye witness statement victim upstairs on goods vehicle which was standing near transformer center and hold middle live DO fuse of transformer center and try to hit the nearby DO fuse with help of iron pipe at that time big spark occurred and get electric shock and victim fell down on road and met with non fatal accident . On dated 29.01.2016 around at 10:40AM, when victim Miss Jaguben Karanabhai Kodiyatar passing on her bicycle through the village road near the border area of village Sangawada-Diwasa, during this 11kv Diwasa Ag feeder live conductor snapped on ground and she came in contact with charged conductor and electrocuted on the spot. After accident site visit of by DE- Madhavpur and after primary investigation it was found that, there is one Jina Manda valu Ag transformer near accident site and one squirrel was found dead on ground near this transformer. And after verifying the detail in 66kv Shil sub station, it was found that at 10:30AM transient tripping was recorded in 11kv Diwasa Ag feeder, after this tripping on duty operator charged the line at 10:35AM and line was charged without second tripping and remained charged till the person name Sh. Ranabhai Galchar informed the on duty operator about the accident and snapping of conductor. So from the preliminary investigation it is found that conductor broken due to heavy fault current passed due to squirrel touched the live TC DO in which there is Iron part in between two horns & hence Squirrel is get electrocuted and after first transient tripping, on duty operator charged the line and line remained charged even though line conductor had already been snapped on the ground. Before that no any infromation regarding said accidnet either from Police Department NOR from legal heir of Victim. After initimation from Motor Accident trubunal Gondal court, SDO Gondal R-1 rushed accident place and prilimany investigated the matter as below :-
As per statement of eye witness, On 01.02.2016, at 14:00 a Toros-truck(GJ-10-V-9396) of owner Sh Natubhai Harubhai Adatiya was parked near Nagarik Bank-Gondal in the compound of New Marketing Yard – Gondal under 111KV RDC Urban Feeder at above location. After full loading of onion bories, the victim climed at the top of loaded truck for binding of goods with cotton rope. When he through cotton ropes from his hand, he came in contact with 11KV RDC Urban feeder and electrocuted.
It is also noted that at said location the ground clearance is approximately 19ft as per measured by SDO. So, accident might occurs due to negligincy of Truck Driver and Victim. Further, investigation is under process.

On the site there is a gurading between the psc pole and girder pole.One squirrel is shorted between the psc pole top andV cross arm of ll KV Chanpa jgy emanating from 66 kv Thikariyali S/S. Due t-o this po*", supply passed through guarding and also from girder pole and during this time t'i"ti. touch the girder pole and resulted in to above accident.

A 1-PH. SERVICE WIRE, GOING FROM T.C. POLE TO CONSMER METER BY CROSSING THE ROAD, WAS BROKEN BY UN-KNOWN VEHICLE. THE LIVE END OF THIS SERVICE LINE WAS LYING ON THE LAND NEAR THE T.C. STRUCTURE. AT THAT TIME A BULL WAS PASSING NEAR THIS T.C. STRUCTURE & CAME IN CONTACT WITH THIS LIVC END OF SERVICE LINE & DIED. DETAIL INVESTIGATIN WIL BE DONE & REPORTED SOON.

On dated 04.01.2016 time approximate 3:30 pm a hydraulic dumper (GJ-03-AZ08515 ) was parked below HT line of 11 KV Reliya Ag feeder. While dumper was off loaded by pushing it upper side and the top most part of the back side of dumper touched the 11 KV line live wire at the same time driver was coming down from his cabin to ground so the live leakage path was observed and as a result he was electrocuted and human fatal accident occurred.

When the owner have depasture their buffalos,near the 11 kv D O switch of HT line of Porda Jgy feeder.One buffalo was iflinging body with stay wire,at that time buffalo horn fixed with stay wire and by trying to eccape from it at that time stay wire free from land with anchor rod of stay wire which fixed in the horn of buffalo.And buffalo run around D O switch D P structure at that time stay wire touched with jumper of H T line of porda jgy therefore current passed from stay wire and buffalo got electrocuted.
