After visiting site today morning found that while victim was doing something illegal work on above location area & came in contact with live 11 kv line at upcoming jumper on 5 kva tc.Hence leakage current passedthrough his body & he was electrocuted. 3	RJR	Village - Jetpur,  N. G. Patodiya ALM	09.04.2015	NFHD	Shri T.J. Sarvaiya - LM, N.G. Patodiya – ALM and Shri G.B. Jani – ALM was allotted with contractor Sh. Devshi bhanji rathod for dismantling  of 200 KVA T/c of N.K. Enterprise industries. At first shri T.J. Sarvaiya has taken the line clear of 11 KV Bhadarkantha feeder on which this T/c is to be dismantled and by making earthing on both side of T/C centre and removed the DO fuses of T/c. Then he allowed to contractor and co-worker to dismantled  the T/c. from DP. Accordingly contractor has provided chain coppo  on DP channel and sealing was provided on lifting leg of top plate of T/C with the help of long iron bolt. While tilting T/c from DP and put it on tractor trolley right side sealing slip  due to escape bolt from hole of lifting leg and T/C fallen on tractor trolley. During  this victim Shri. N.G. Patodiya standing on tractor trolley and while pushing transformer body, at that time occurrence took place and transformer fallen on tractor trolley and conservator tank beat on his head. So helmet was broken and removed from his head and his head is injured. Also fracture is happen on his right leg. In this way Non fatal mechanical accident is occurred. 4	RJR	Village - Kundech, Sh. Kishore Dhana Makvana	24.03.2015	FHO	As per message known from “SANDESH” News Paper on dated 08-04-15, a fatal accident occurred to Shri Kishore Dhana Makvana near Gaushala at village  Kundech. After site visit it is found that the victim was doing sainting work with iron rod length 16 feet for godown of gaushala on dated 24-03-15 and during the work anyhow rod came in contact with the induction zone of live part of conductor of 11 KV line which is passes near to the Gaushala having Horizontal distance of 6.5 ft from existing line. So victim got electrocuted and fell down on the ground at the same time and nonfatal accident occurred at site which is converted in to fatal at later on dated 03.04.2015. Saydaben bhangda kaliya was going from the farm of shri Raghavjibhai Muljibhai Sojitra to Rameshbhai Nanjibhia ,there is one Drum Stick tree on the way of farm . 11 kv targhadi AG feeder is passing beside this tree .She was picking Drum stick and its leaves from drum stick tree for evening meal purpose. Vinchhiya As per statement of eye witness Victim was came in contact with snapped LT conductor of Monpar Village TC and he got Electric shocked. As per site visit of DE Vinchhiya there was Two LT Line conductors were snapped from LT pole of Monpar Village TC. Near LT pole on the ground shades of blood and one cloths (“Lungi”) found which cloth on also shades of blood found.  At accident place no any connection given from this LT pole.  So, further investigation is under process. 22	RJR	Village - Khakhana KANUBHAI MAVJIBHAI GEDIYA	21.09.2015	NFHO	As reported by the SDO- Kuvadava As  5kva T/C of village Khakhana was failed, so contractor of failed transformer  replacement is come for replace same failed TC of village Khakhana on dt.  21.09.15 Time (app.) 16.30 with total 03 no of labor. Arrangement of shut  down of power supply is done by depute PGVCL employee Sh.M.G.Sangala  (E.A.) & he was standing at AB switch located at village Khakhana to cut down  power supply of 11kv Jiyana JGY feeder for replacement of failed TC. However victim Sh. Kanubhai Mavjibhai Gediya & other two labors reached at location of failed TC at village Khakhana & victim has without verifying about shut down of power supply, shorted/earthed of 11kv line clime pole, (even though his co-worker & local villagers has informed him about power is on) & come in induction zone of existing 11kv Jiyana JGY feeder (No tripping come in feeder) & getting electric shock. Then after victim is referred at Hospital, Rajkot & after preliminary treatment he was discharge from Hospital on same day. 23	RJR	Village - Dungarpur THE BUFFALO OF SHRI DINESHBHAI BHIMJIBHAI RATHOD	21.09.2015	FA	As per par of DE Sardhar, There is continuous rain from last 3 days due to low depression effect at sardhar Sdn, as per owner statement on Dtd. 21.09.2015, He went to his AG land with Two buffalo’s while returning to home at evening approx 17:55 PM one buffalo was passing near 11 KV pole it is erected in front of his house and that time rain is continue and when buffalo passes near this pole and suddenly felt down on the ground and died. DE sardhar has taken site visit at same time during site visit there is no any leakage current was found from this HT pole. As per owner statement the accident took place due to leakage current in G.I. wire but the actual reason may come to after post mortem report. 1	MRB	At Lalpar, Ta Morbi, Chaturbhai Jivajibhai Ninama	18.4.2015	NFHD	While the victim was climbing over Girder DP structure to attend complaint involving rectification of jumper, the DP structure began to bend down to ground. The victim then swiftly reacted by speedily climbing down the structure & when he was about 10FT high he jumped from the pole to the ground. Eventually the DP structure was completely grounded as the pole broke. The victim in the process sustained fractured in his left wrist. The doctor has advised him about a month’s rest. 2	MRB	At Lalpar, Ta Morbi, Babulal Parmeshwararam Chaudhari	6.6.2015	FHO	The victim climbed over truck cabin to pull off cargo plastic cover with the help of iron pipe. In the process the rod/ pipe must have touched bare conductor of the feeder & thus electrocuted. There was no tripping observed in the feeder. 3	MRB	At Mahendranagar, Ta Morbi, 1 Buffalo of Jayesh Devabhai Goltar	15.6.2015	FA	The victim buffalo was traversing flash flood stream body (Vonkalo) below LT line span emitting from transformer# GHU/BELA/10/L-15/DT03 of 11KV Bela AG feeder. During the time of the accident prevailing bad weather & heavy wind snapped the LT conductor which then fell on the victim buffalo & thereby the buffalo electrocuted. 4	MRB	At Nichi Mandal, Ta Morbi, Ratilal Jayantibhai Sarasavadiya	25.6.2015	FHO	The victims were in the process of shifting connectivity’s antenna pipe from its current clamp type fixture to fabricated stand type fixture over the toilet (about 15FT high from the ground level) inside the campus wall of M/S Sayona Plastwood. The antenna pipe was 19FT & 9 inches long & was quite heavy. When the victims were removing the pipe from the clamp structure out of sheer heavy weight & windy weather the pipe imbalanced & fell on the live bare conductors of 11KV Ramnagar JGY feeder passing adjacent to the campus wall. The victims thus electrocuted. The victims were well inside the campus wall of M/S Sayona Plastwood. Accordingly said work was started and during the work, at about 01:30 PM , Victim Mr. Vikrambhai Danabhai Khavdu climb on the (PIPAL) Tree located between Pole Loc:-BOK/RIN/131/L052 and BOK/RIN/131/L053 and stand on one tree branch for cutting the obstructed tree branch in 11 KV Rinawada AG feeder. Due to the broken of the tree branch on which victim was standing, he felt down on the ground from the height of approx. 6 Ft. There was stones of the verandah of the farm were on the ground where  victim felt down . So, his left leg felt on the stone and  resulted in injury-fracture  in the left leg above the ankle. The vicitim climbed the pole which was double power location of 11 KV Subhasnagar and 11 KV Gaytrinagar Feeder - to provide gaurding. Out of these two feeder 11 KV subhasnagar feeder was under planned shutdown and 11 kv gaytrinagar feeder was live. the vicitm might have tried to remove thread from that 11 KV line pole and come in to contact of live power and got electrocuted. It is further to state that there was no tripping on line observe at SS end on GMDC feeder between 04:00 pm to 8:00 pm. 1	BTD	At Village Dharuka  , Shri Bhupatbhai Jatabhai Tank	01.05.15	FHO	At Dharuka village Ex. 63 KVA tc of ag group of Hussainbhai is located near ag land of Bhupatbhai Jatabhai Tank. Shri Bhupatbhai done his ag farm fancing with the use Barbad wire which was touched directly to DP structure of this AG group tc. As this fatal accident occurred  at 60 to 65 feet away from this AG TC & looking to the site, first it is observed while he was diverting water in his ag land due to any un avoided circumstances he came in contact of leakage electrical current and immediately electrocuted. As this leakage of current flows from transformer center or LT wiring or elsewhere hence the exact cause of electrical accident required thoroughly investigation. 2	MRB	Vipul Jayantilal Mistri; Mahendranagar;  Ta Morbi	15.1.16	NFOS	The top conductor of 11KV Mahendranagar Ind Feeder snapped from between Location# Bharatnagar/M’nagar/072 (Pin point 11Mtr Girder Pole) & Location# Bharatnagar/M’nagar/073 (Transformer on 11Mtr Girder Pole). The snapped conductor then fell on the victim standing immediately below it & thus the victim met with the non-fatal accident. Primary analysis reveals that the technical reason for this snapped conductor could be the kite that entangled on the conductor of the said feeder between location# 82 & location#83, which then momentarily may have shorted 2 conductors. The land surface immediately on & around PSC pole was dry & the wet land was about 10 FT away. The leakage current, created due to pin-fault, may have passed onto wet land where these buffaloes where drinking water either through Babul tree’s green branch touching the guy wire above guy insulator or through the dry earth surface area below PSC pole.  As there were sparking/ black marks on the branch of the tree which touched the guy wire & also over iron rods of PSC pole & that this babul tree’s roots sprayed across wet land, the first theory that the leakage current passed onto wet land through green branch seems most probable. The guy wire uprooted from the ground due to sheer force of the collision of the victim animal. The guy wire was of 11MTR girder pole through which highway crossing was given. The uprooted guy wire touched live wire touched live conductor of the same 11MTR girder pole. The conductor contacting the guy wire was tied to this girder pole at normal PSC pole line height & this conductor was then raised by providing jumpers to the top of this 11MTR girder pole so that necessary clearance could be maintained for Highway crossing. The guy wire thus became live as the guy insulator was provided between the heights of these 2 levelled conductors. The buffalo in contact with this guy wire thus electrocuted. As per his information the accident was occurred at 8:30 am in the morning. The victim was working in the construction building and when he lift up the iron rod , the rod touched the line and he got electrocuted. When we reach the site for investigation we asked in the Sub Station about the 11KV Chhathibari Feeder Tripping. But there was no Tripping in the Feeder. So it is not clear that how he got electrocuted. We asked in the Bhuj City 1 Line office also about any call for accident. But no one had called for any accident. 40	PBR	Sangavada-Diwasa Simado , Miss Jaguben Karanabhai Kodiyatar	29.01.16	FOS	On dated 29.01.2016 around at 10:40AM, when victim Miss Jaguben Karanabhai Kodiyatar passing on her bicycle through the village road near the border area of village Sangawada-Diwasa, during this 11kv Diwasa Ag feeder live conductor snapped on ground and she came in contact with charged conductor and electrocuted on the spot. After accident site visit of by DE- Madhavpur and after primary investigation it was found that, there is one Jina Manda valu Ag transformer near accident site and one squirrel was found dead on ground near this transformer. And after verifying the detail in 66kv Shil sub station, it was found that at 10:30AM transient tripping was recorded in 11kv Diwasa Ag feeder, after this tripping on duty operator charged the line at 10:35AM and line was charged without second tripping and remained charged till the person name Sh. Ranabhai Galchar informed the on duty operator about the accident and snapping of conductor. So from the preliminary investigation it is found that conductor broken due to heavy fault current passed due to squirrel touched the live TC DO in which there is Iron part in between two horns & hence Squirrel is get electrocuted and after first transient tripping, on duty operator charged the line and line remained charged even though line conductor had already been snapped on the ground. After initimation from Motor Accident trubunal Gondal court, SDO Gondal R-1 rushed accident place and prilimany investigated the matter as below :- As per statement of eye witness, On 01.02.2016, at 14:00 a Toros-truck(GJ-10-V-9396)  of owner Sh Natubhai Harubhai Adatiya was parked near Nagarik Bank-Gondal in the compound of New Marketing Yard – Gondal under 111KV RDC Urban Feeder at above location. After full loading of onion bories, the victim climed at the top of loaded truck for binding of goods with cotton rope. When he through cotton ropes from his hand, he came in contact with 11KV RDC Urban feeder and electrocuted. It is also noted that at said location the ground clearance is approximately 19ft as per measured by SDO. So, accident might occurs due to negligincy of Truck Driver and Victim. THE LIVE END OF THIS SERVICE LINE WAS LYING ON THE LAND NEAR THE T.C. STRUCTURE. AT THAT TIME A BULL WAS PASSING NEAR THIS T.C. STRUCTURE & CAME IN CONTACT WITH THIS LIVC END OF SERVICE LINE & DIED. Laljibhai Vaghubhai Barad	04.01.16	FOS	On dated  04.01.2016 time approximate 3:30 pm a hydraulic dumper            (GJ-03-AZ08515 ) was parked below HT line of 11 KV Reliya Ag feeder. While dumper was off loaded by pushing it upper side and the top most part of the back side of dumper touched the 11 KV line live wire at the same time driver was coming down from his cabin to ground so the live leakage path was observed and as a result he was electrocuted and human fatal accident occurred. 94	SNR	Patdi, Buffalo of shri Motibhai Narayanbhai Khambhala	28.02.16	FA	When the owner have depasture their buffalos,near the 11 kv D O switch of HT line of Porda Jgy feeder.One buffalo was iflinging body with stay wire,at that time buffalo horn fixed with stay wire and by trying to eccape from it at that time stay wire free from land with anchor rod of stay wire which  fixed in the horn of buffalo.And buffalo run around D O switch  D P structure at that time stay wire touched with jumper of H T line of porda jgy therefore current passed from stay wire and buffalo got electrocuted.
