Mansukh Bachu Bhalodia, Maji Sarpanch of Moti Paneli on Dtd.18.09.13 at about 11:00 a.m. that a fatal accident occurred private premises of Dinesh Chhagan Kardani farm and accordingly site investigation was carried out and it was found that due to non-availability of power supply of 3 phase motor. Victim was repairing their starter at there farm- private premises situated at their Electric Room-“Ordi” near well. He came contact with live part of “Y” (Middle) phase wire. During rewinding of steel rope attached with pulley at top of mechanism having height 22 feet. At the time of rewinding process of steel rope victim pulling the rope and gone back foot for straightening of rope to rewind and it came in contact with 11 KV line conductors which is passing over through it. So victim got electrocuted & other three co-workers get electrical jerk. Hence electrical accident occurred.

Having been injured, he was shifted to Wankaner government hospital wherein the doctor on duty declared him dead. There was no tripping observed on the feeder. On being informed about the accident on 06.09.2013 by local police station, the DE Sh N I Prajapati visited the site at 10:00PM on 06.09.2013. The ground clearance of the line is about 6.55Mtrs & that from the bathroom ceiling is 1.5Mtrs. Since bathroom structure has been recently built by extending porch (only its colour work was to be done), the feeder line has thus directly come above it. The DE has confirmed issuance of the notice on same subject to concerned party/ defaulter.

He went to the last bench in the classroom, bench was fitted with metal frame. Behind the said bench there was wiring board just at 1.5feet height from the flooring. LAN Board for connecting computer and other switch board was also lying on the floor. Near this bench a phase wire of this wiring was open. Also the walls of class room were wet due to rain. When victim has tried to shift the wooden bench having metal frame, it touched to live phase wire and the student (victim) got electric shock.

So dropper of LT Line as well as AB Cable is at such a distance where nobody can reach & touch the Line easily & accidently, so it is clear that victim came in contact with the live portion of dropper of LT Line with his personal malafide intention & met with the accident.

Non-Fatal Human accident occured due to vicitm got electric shock by came in contact with Metallic Fencing of Mobile Tower Premises which might be electrocuted via G.I.wire of Service cable of Connection. One end of G.I. bound with Service Pipe and other with M.S.Angle of the Pole. Before Pipe,G.I. touched with Fencing at the service conn. End. In the very first span of LT Line, there is AB Cable running in parallel with single PVC Coated wire(which was provided only for Streetlight Phase) with Guarding. In this span, G.I.Wire of guarding might have been intertwined with coated PVC Wire of street light Phase through kite threads. Due to kite threads overlapped on PVC Coated wire tightly, insulation may got deteriorated & as the atmosphere is also moisturized due to rain, there may be leakage current flow from this coated wire to guarding and then to G.I. wire of service connection. And as before termination at the other end, this G.I. was touch the fencing, it also got electrocuted.

A dropper wire, passed through shackle insulator, touched to C clamp due to failure of insulation where it is in contact with sharp edge of C clamp. So, phase wire earthed to the ground through C clamp. As it was raining and water was filled near LT line pole, step potential was created around the pole. A buffalo got an electric shock. While tracing the reason along with evidence found at the site and referring to the statements of the eye witness, it was found that an unauthorized lunger was extended from LT of Village DTC of Sauka to the pole of accident and the service line used for the same
Unauthorized use of electricity found to be broken and lying at the ground where direct contact with buffalo was found and as a result accident occurred. The cause of accident is unauthorized use of electricity in the Meledi Mataji Mandir and same is booked as Non-Consumer by this office.

A human fatal accident occurred to two outsider persons at Nilkanth School-Limbdi on dt.25.07.13. At the time of accident the victims were doing the masonry work at the Nilkanth School. At the time of visit of accident place it was found that the extension work for school building was going on and plaster work is in progress. During the finishing of the plaster the victims may have extended his aluminium tool for such finishing and the same came in direct contact with the side phase passing nearby the building which was about 3.25 feet away from the building. The victims were standing on the temporary wooden structure constructed for such mason work at some height. At the time of site visit some cement spot was found on the wire towards the building.

On Pole no MET/MET/72 and MET/MET/73 pin binding of one pin insulator is removed due to which one phase conductor loosen down up to three feet from the ground but the way how pin insulator bindings are found removed and inspite of several tree branches below the conductor span and even then instead of conductor falling on tree braches and to get mess in it, it has fallen near to ground, it shows that this is fabricated reason to misguide PGVCL.If conductor would have really snaped then it is to fall on tree branches and would have messed in tree branches and would have hanged in tree braches insted of falling up to near ground level. Really cause of accident may be some internal wiring fault at some where or due to mal handling and tampering with PGVCL line network.
